Pharmacy and Medication Safety: Preventing Errors and Ensuring Patient Well-being

At the June 28, 2018
Pennsylvania State Board of Pharmacy (Board) Meeting, the Board voted to waive
the requirement that Pharm.D. Graduates of schools of pharmacy accredited by
the Accreditation Council for Pharmacy Education (ACPE) earn at least 500
intern hours outside of their academic program. This means that the 1,500
intern hours required for licensure as a pharmacist may be earned through an
ACPE-accredited school of pharmacy’s Pharm.D. Program and these graduates are
no longer required to earn 500 intern hours outside of the school’s academic
program. Vazquez is one of eight Camden high school students selected in their junior year to be part of the district’s first pharmacy technician program.

Although the main problems of irrational drug use in Jordan and Syria were almost the same, they varied in the percentage of occurrence. These problems included excessive use of antibiotics and antidiarrheals, overprescribing of nonsteroidal anti-inflammatory drugs, self-prescribing medications, common use of antibiotics to treat minor upper respiratory infections, and prescribing by trade names. In addition, pharmaceutical care free days were conducted by pharmacy students to help people take responsibility for their health, educate and counsel patients about medications, and educate them on how to manage their chronic diseases. A community pharmacy is a healthcare facility that provides pharmaceutical and cognitive services to a specific community. From independently owned pharmacies to corporately owned chain pharmacies, a variety of pharmacies are in operation. In some developing countries in Africa and Asia, the terms “drug outlets,” “retail drug outlets,” “retail drug shops,” and “private pharmacies” are commonly used.

In America, Benjamin Franklin took a pivotal step in keeping the two professions separate when he appointed an apothecary to the Pennsylvania Hospital. Separation of prescribing and dispensing, also called dispensing separation, is a practice in medicine and pharmacy in which the physician who provides a medical prescription is independent from the pharmacist who provides the prescription drug. In the US, pharmacy technicians perform their duties under the supervision of pharmacists. Although they may perform, under supervision, most dispensing, compounding and other tasks, they are not generally allowed to perform the role of counseling patients on the proper use of their medications. Some states have a legally mandated pharmacist-to-pharmacy technician ratio. The boundaries between these disciplines and with other sciences, such as biochemistry, are not always clear-cut.

A pharmacy (also known as a chemist in Australia, New Zealand and the British Isles; or drugstore in North America; retail pharmacy in industry terminology; or apothecary, historically) is where most pharmacists practice the profession of pharmacy.

An inventory of current stock quantities and expiry dates should be done before each order. Stock cards give a theoretical figure of stock quantities, but actual quantities of each product should be verified (physical stock). An inventory may only be easily done if the pharmacy is correctly arranged.

BROOKLYN’S ONLY PHARM.D. PROGRAM

The State of Maryland pledges to provide constituents, businesses, customers, and stakeholders with friendly and courteous, timely and responsive, accurate and consistent, accessible and convenient, and truthful and transparent services. Persons dispensing drugs should be able to give patients the information they need. Interpreters are needed if several languages exist in the same region. The management of the pharmacy should be entrusted to a single person having received adequate training. This person is the only person possessing keys to the pharmacy and narcotics cupboard and is helped by one or more assistants, depending on the workload. Once standard therapeutic regimens and lists of drugs and supplies have been established, it is possible to calculate the respective quantities of each product needed from the expected number of patients and from a breakdown of diseases.
