Prescription Dermatological Medications: Treating Skin Conditions and Disorders

Our work extends across opportunities within the Faculty of Medicine and connections with other University schools and faculties. We collaborate nationally and internationally with academics and clinicians to develop, deliver and implement research that has an impact on policy and clinical practice. Our active patient and public involvement members help ensure our research is both inclusive and relevant to patients’ priorities. Our researchers are driven by a desire to create better treatments and diagnostics for their patients, specialising in world-leading patient-focused translational research. We’ve conducted many clinical trials, including being a leading centre for the COVID vaccine trials. Modern scientific biomedical research (where results are testable and reproducible) began to replace early Western traditions based on herbalism, the Greek “four humours” and other such pre-modern notions.

This internalization is most evident in diseases such as chronic lymphocytic leukaemia (CLL) and mantle cell lymphoma (MCL) which remain relatively unresponsive to rituximab. They have now demonstrated that the inhibitory FcγRIIb on target B cells promotes rituximab internalisation and that the level FcγRIIb expression is inversely related to the rate and extent of rituximab internalisation. Early results show that in MCL patients, high FcγRIIb expression may actually predict less durable responses following rituximab-containing regimens.

This programme has now produced agonistic reagents recognising OX40 and 4-1BB, for clinical use, and is also providing selected reagents for a consortium of four cancer immunology centres and four SMEs across Europe focused on developing agonistic antibodies for use in difficult to treat cancers. Finally, the Antibody and Vaccine Group are working with collaborators in the USA (Dr T Bullock (Virginia) and Celldex Therapeutics) to develop immunostimulatory monoclonal antibodies against the TNFRSF molecule CD27. Our scientists have made significant contributions to the development of new treatments for asthma and IPF, contributed to new clinical and policy guidelines for disease management and provided a better understanding of the natural history of asthma and allergic diseases. Our understanding of these diseases has been substantially enhanced by the array of in vitro models of lung disease developed by Southampton scientists. These models include lung cells and tissue donated by our diverse and committed volunteers who participate in our clinical cohort studies. We respond to stakeholder need and the local, national and international healthcare ecosystem to drive and support meaningful change.

Other approaches may be tried if a child doesn’t respond, Sukhodolsky says, adding that some children need more intensive outpatient services or even inpatient treatment. “A life of emotional intensity doesn’t feel good so our focus and treatment goal is to help the child feel better and not suffer,” he says. Nobody likes to feel angry, but we all experience the emotion from time to time. Given that many adults find it hard to express anger in ways that are healthy and productive, it’s unsurprising that angry feelings often bubble into outbursts for children.

The second question has been whether Omicron and its subvariants are more likely than their predecessors to cause severe disease. While there is more to learn about the latest variants, experts are hoping prior immunity will be of some help. The original Omicron caused a record number of cases, but while it has also caused its share of hospitalizations and deaths, factors such as lengths of hospital stays, ICU admittance, and death were “lower than during previous pandemic peaks,” according to a CDC report in January 2022. According to the CDC, the Omicron variant spreads more easily than the original SARS-CoV-2 virus and the Delta variant. In its early days, the variant caused an alarming spike in COVID-19 cases in South Africa—they went from 300 a day in mid-November 2021 to 3,000 a day at the end of that month.

In the first months of 2022, an Omicron subvariant called BA.2 began to spread even faster than other Omicron subvariants, followed by BA.4 and BA.5, only to be outdone by the BQ subvariants. Early reports from South Africa indicated that most cases were mild—and that symptoms for this variant seemed to be different. “The reports show that patients in South Africa—many of whom were young—have had severe fatigue, but no loss of taste or smell,” says Lauren Ferrante, MD, a Yale Medicine pulmonologist.

Immunostimulatory monoclonal antibodies became newsworthy when the drug ipilimumab proved effective against metastatic melanoma and was approved for patients by the Food and Drug Administration. They have gone on to develop an engineered anti-CD40 monoclonal antibody (ChiLob7-4), which has now completed phase I trials. It appears that these reagents work primarily by binding to CD40 on antigen presenting cells such as dendritic cells, but probably also B cells, and activating them to a level that allows the generation of effective tumour-specific T cells. Often this is achieved without the need for T-cell help, allowing responses in the absence of the rare helper epitopes. Dr White, also part of the Southampton team, has recently shown a critical role for FcγRIIb as a promoter of the agonistic activity of certain anti-CD40 mAb.
